Dear Family,

The purpose of this letter is to communicate to you my irrevocable desire
to be an organ and tissue donor. I have researched this matter thoroughly
and seriously considered my decision. I want to give the gift of life and
enhance the quality life of as many people as possible upon my death. But,
I will need your help to make sure that my wishes are carried out. This is
because, you, as my next-of-kin, will be called upon by the medical
professionals to provide consent before donation can occur.

Also, I have documented my intention to be an organ and tissue donor in
other ways, such as my driver's license and/or a donor card that I carry
with me. However, as I stated above, it will require your consent for my
wishes to be carried out at the appropriate time.

I know that it would be a difficult time for you, but families of donors
have said that it helped them greatly to know that they were carrying out
the wishes of their loved oneand helping others. I hope that by communcating
my wishes to you now, you can share with me the knowledge that together
we can make a difference by saving or improving the lives of up to 50
people and their families.
